Rates
$200 per 1-Hour session and $175 per 45-minute session.
Insurance
Dr. Kalin is an OUT-OF-NETWORK provider for PPO insurance (not HMOs) such as Blue Cross, Care First, Aetna etc. Dr. Kalin is a Non-Network Tricare provider for active and retired military.
Therapy sessions are often covered in part by your health insurance or employee benefit plan. Please check with them directly for details.
Dr Kalin is an Opt-Out provider and does not participate in Medicare or Medicaid. However, patients can submit to their secondary PPO insurance for Medicare.
Payment
Payment accepted by check, credit or debit card, some electronic apps, and cash. Payment must be made before the scheduled session. Dr. Kalin will then email a statement (invoice) for client's submission to their insurance provider.
For returned checks, the payment must be immediately replaced with cash, credit card, or electronic app. This will include any returned check fees which are usually between $25 - $50. This same policy applies to NSF fees for credit or debit card payments.
Cancellation Policy
At least 48 hours in advance is required (except for medical emergency). Otherwise full cost of the session will be charged.
